Medical FSA and Dependent Care FSA are different, removed confusing information.

Specifically, the plan must have: (1) a minimum annual deductible ($1,300 self-only / $2,600 family for 2015-2017), and (2) a maximum annual out-of-pocket expense ($6,550 self-only / $13,100 family for 2015-2017). The table of annual values is also on the Wikipedia page for high-deductible health plan.
